Peter Quinlin (Counsel for Ms Dhu’s family): This was a typical night, wasn’t it? Or a typical shift? 
Constable Matier: Yes. It was. Yes. Being a Monday it’s—typically very busy with prisoners.

Quinlin: But also, it’s…there’s nothing really unusual about, can I suggest, the lack of attention given to Ms Dhu here. It’s pretty typical of the way in which the lookup operated? 
Matier: Yes, yes. I agree. Yes.

Ms Dhu’s time of death was recorded at 1:39pm, by which time officers from WAPOL internal affairs were already on a flight from Perth. At the end of their investigation, 3 managerial notices and 7 letters of correction were issued which indicated that officers had failed to comply with police regulations. During the inquest, it was clear that few officers had paid much, if any, attention to these letters. None of the officers involved were subject to any meaningful consequence.
